Dear Parent/Athlete,

Students planning to participate in high school interscholastic sports, cheer, or poms must have a current athletic physical on file prior to the first practice of the 2023-2024 school year. 

An athletic physical may be scheduled with your family doctor, or be done at Princeton High School on Wednesday, May 10, 2023. Sports physicals at PHS are provided at no cost this year by OSF doctors, nurse practitioners, and nurses as a community service. All physicals are good for 395 days. 

Each student must complete the IHSA Participation Examination Form (Below) which includes medical history and contact information.  This form must be completed and signed by both parent and student/athlete and returned to PHS by Friday, May 5th  to participate in the athletic physicals at the high school.  The form is attached below. Forms are also available in the student affairs office.

 Student athletes should arrive at the Main Entrance door #1 according to the schedule below. 

                 Athletic Physical Schedule – Class for school year 23-24

                      3:45  Seniors    4:00  Juniors    4:15  Sophomores

Students will be seen in order of check in; last year everyone was done around 5:00 PM.
